SELF CARE TEMPLATE for INSPIRES.
Purpose of group involvement: To practice skills and process information that has been presented throughout treatment so that new skills can be integrated into practical uses of self-care throughout our
recovery.
Group Topics: Self-care Assessment.
Description of group including intervention/technique: Consumers practiced skills that were learned throughout their recovery so far, and discussed how to integrate them into practical life skills that are useable and actionable. Consumers shared and discussed the self-care assessment and the skills that they have learned throughout treatment and how it has improved their ability to take better care of themselves and process their emotions through actions of self-care.
Treatment Plan Objective/Treatment Strategy: Consumer will practice and discuss the things they learned through treatment after filling out the self-care assessment and discuss different ways they can better practice self-care and better process their emotions through positive actions.
Consumer's response to treatment intervention: Interval history including current symptoms/functioning (if substance abuse, include craving scale/potential for relapse or other high risk factors): reported their mood as good, reported no recent worries, concerns, or triggers. He rated his cravings, anxiety, and depression on a scale from 1 to 10. He rated his cravings at a 1, anxiety at a 1, depression at a 1, and did not disclose SI/HI.
Consumer will continue inpatient treatment and plans to continue long term inpatient treatment.
